CHAPTER 3 — The Price of Silence

No one spoke.
Not Anna.
Not the nurses.
Not even Luca Marino.
The surgeon's question lingered in the room like smoke after a gunshot.
Finally, Luca answered.
"I don't know."
The words surprised everyone.
Especially himself.
For years, people had expected certainty from him.
He always knew who had betrayed him.
Who needed protecting.
Who needed punishing.
Today...
He knew nothing.
The surgeon folded his arms.
"I've treated cancer for thirty-two years."
"I've seen people survive impossible odds."
"And I've seen perfectly treatable cases become fatal because patients had no reason left to fight."
He looked toward Anna.
"Medicine removes tumors."
"It doesn't heal loneliness."
Anna stared at the ceiling.
Every sentence felt painfully true.
Later that afternoon, Luca found himself standing in the tiny Williamsburg apartment Anna had rented after leaving him.
He had never been there before.
She never told him the address.
He didn't need her to.
His people had known from the first day.
He simply chose not to interfere.
Now he wished he had come.
Not to bring her back.
Just to see how she had been living.
The apartment was barely four hundred square feet.
Secondhand furniture.
A leaking kitchen faucet.
Paint peeling near the window.
The refrigerator held eggs, yogurt, and leftover noodles.
Nothing else.
He opened one cabinet.
Inside sat a stack of unpaid medical bills.
Another drawer held rejection letters from insurance companies.
Then he found a small notebook.
It wasn't hidden.
The first page read:
Things I learned after leaving.
Luca hesitated.
Then opened it.
The first entry shattered him.
Day 3.
I bought my own coffee today.
It tasted terrible.
But nobody bought it to make me stay.
The second entry.
Day 41.
I fixed the sink myself.
It took six YouTube videos.
I've never felt more capable.
Then...
Day 109.
I still love him.
I just can't survive inside his silence.
Luca closed the notebook.
His hands were shaking.
Not from rage.
From regret.
That evening he returned to the hospital.
Anna was awake.
She immediately noticed the notebook in his hand.
"You read it."
"I did."
"You invaded my privacy."
"I know."
She expected him to defend himself.
He didn't.
Instead—
"I'm sorry."
She blinked.
"What?"
"I'm sorry."
"I should've asked."
"I should've listened."
"I should've..."
His voice broke.
"...learned how to be your husband instead of your bodyguard."
Anna stared at him.
Three years together.
She had never once heard Luca Marino apologize.
Not once.
Before she could answer—
The monitor beside her bed suddenly erupted.
BEEP!
BEEP!
BEEP!
Anna grabbed her abdomen.
Pain exploded through her body.
Doctors rushed in.
"Nurse!"
"Call the OR!"
"Blood pressure's dropping!"
Luca reached for her hand.
Anna squeezed it with surprising strength.
Then whispered only four words.
"Don't leave this time."
The operating room doors slammed shut between them.
For the first time since he was nineteen years old...
Luca Marino dropped to his knees.
And prayed.
Two hours later...
The lead surgeon finally stepped outside.
His surgical mask was stained.
His eyes were exhausted.
Luca stood instantly.
"Tell me."
The surgeon removed his gloves slowly.
"The tumor..."
He stopped.
